What are your two options for one missing tooth?

A bridge and an implant both put a fixed tooth back in the gap. Neither comes out at night, and once it is done nobody can tell which one you chose when you smile. The difference is support: a bridge borrows it from the teeth on either side, while an implant gets its own root in the jaw.

A bridge is three crowns joined in a row. The middle one, the pontic, sits on the gum where your tooth used to be. The two outer ones are cemented over the neighbouring teeth, which first have to be trimmed on every side so the crowns fit over them. An implant is a titanium post placed in the bone by our in-house specialist team. The bone grows onto it over a few months, and then a single crown is fixed on top. The neighbours are not touched.

Most people who reach this decision in my chair already have two quotes on their phone, usually one for each, and the numbers look very far apart. They are closer than they appear, and the gap closes further once you count what happens after year ten. The most common case I see is a lower first molar, lost to a cracked tooth or a root canal that was never crowned, with two perfectly healthy premolar and molar neighbours on either side.

One thing that trips people up: a bridge quoted at “₹5,000” is almost always a per-unit price. One missing tooth means three units, so it becomes ₹15,000. Ask every clinic for the total, not the unit rate. What do you pay over 10 and 20 years?

Over ten years, both options usually cost what they cost on day one, because most of each are still working. In the largest review of these restorations, about 89% of conventional bridges and 89% of implant crowns were still in place at ten years. The picture changes in the second decade, when a bridge that needs redoing roughly doubles its bill.

The numbers come from Pjetursson and colleagues’ 2007 systematic review, which found 10-year survival of 89.2% for conventional tooth-supported bridges and 89.4% for implant-supported single crowns. A later review by Jung and colleagues (2012) separated the implant from its crown: 95.2% of the implants themselves were still there at ten years, while 89.4% of the crowns on them had survived. That is a useful detail for budgeting. The root usually outlasts the tooth sitting on it.

The year-20 row is my assumption, not a promise in either direction. Plenty of bridges last twenty years, and some implant crowns need replacing sooner. What the table shows is the shape of the money: a metal bridge redone once ends up level with the implant, and a zirconia bridge redone once overtakes it. A bridge that fails because one of its supporting teeth decays sometimes cannot simply be remade on the same teeth, and then the second round is bigger than the first.

What the table cannot price

Two healthy teeth. Once the enamel is filed off for a bridge it never grows back, and those teeth will need a crown of some kind for the rest of your life, whatever happens to the bridge.

Does a bridge damage the neighbouring teeth?

A bridge does not damage the neighbours dramatically, but it changes them permanently. Each tooth next to the gap is reduced on all sides to take a crown. If those teeth already had big fillings, that is no real loss. If they were healthy and unfilled, it is a cost, even though it never shows on the bill.

This is the part I see from the endodontic side. A tooth that has been prepared for a crown has had heat and trauma close to its nerve, and a few of them lose that nerve over the following years. When it happens under a bridge, the root canal goes in through the crown, and the bridge has to be repaired around it. The 2007 review found that bridges on natural teeth fail mostly for biological reasons, meaning decay at the crown edge and loss of nerve vitality, while implant problems are mostly mechanical: a loose screw, chipped porcelain. Mechanical problems are usually the easier ones to fix.

Cleaning matters too. Food collects under the pontic, and normal floss cannot go through a bridge. You need a floss threader or a small interdental brush every night. Patients mean to do it. By the second year, honestly, most do not, and that is where decay at the edge of the abutment crowns begins. A one-sided cantilever bridge, held by only one neighbour, does worse again: 80.3% survival at ten years in the same review.

Bone loss under a bridge: does it matter?

Once a tooth is gone, the bone that held its root has no work to do, and over months it slowly thins. A bridge sits on the gum and does nothing to stop that. An implant sits inside the bone and is loaded when you chew, which is why bone around a healthy implant tends to hold its shape.

At the back of the mouth this is mostly invisible. At the front it is not. After a few years a small hollow can appear above the pontic, and the false tooth starts to look longer than its neighbours. The second reason it matters is future choice. If you take a bridge now and want an implant in ten years, there may be less bone to work with. We check bone height and width on a CBCT scan, and if a graft is needed it is planned and priced case by case after that scan.

When is a bridge the smarter choice?

A bridge is the better choice when the teeth on either side already need crowns, when there is not enough bone and you would rather avoid grafting, or when health issues make surgery a poor idea. In those cases nothing healthy is being sacrificed, and you have a fixed tooth in about a week.

  • The neighbours are already heavily filled or root-canal treated. They would need crowns anyway. Using them for a bridge means nothing healthy is lost.

  • There is a deadline. A wedding in five weeks is a real reason. An implant will not be finished in time; a bridge will.
  • The budget is genuinely tight this year. A metal bridge at ₹15,000 is a sound restoration, not a second-class one.

When is an implant the smarter choice?

An implant makes more sense when both neighbouring teeth are healthy and unfilled, when the bone is adequate or you are willing to graft, and when you are thinking in decades rather than this year’s budget. It is also the only option that does not tie the fate of three teeth together.

How long does each take: weeks or months?

A bridge is usually finished in two visits about a week apart. An implant takes three to four months from placement to final crown in a straightforward case, because the bone needs time to bond to the implant surface. A graft can add a few months more. Either way, the chair time itself is short.
