What decides it is control: sugar, blood pressure, blood thinners and the bone seen on a CBCT scan. Is there an age limit on dental implants for senior citizens?

There is no upper age limit. The ITI consensus statement on age and medical conditions says advanced age, 75 years or more, is not on its own a contraindication for implants. A healthy 70-year-old often heals as predictably as a 50-year-old. The questions that matter are health control, bone, and whether the implant can be kept clean.

The call usually comes from the son or daughter, often on a Sunday, often from Indirapuram or Noida Sector 62 while the parent lives with them or nearby in Vaishali. “Papa is 71, sugar patient, his lower denture keeps falling. Is it even worth it at his age?” I understand the hesitation. But a 71-year-old in reasonable health may have fifteen or twenty years of eating ahead. Spending them unable to chew a roti properly is not a small thing.

What I look at instead of age: how steady the hands are for cleaning, whether there is memory trouble that would make aftercare hard, how much bone is left after years without teeth, and which medicines are on the list. Implant surgery itself is done by our in-house specialist team; I plan it with them and look after any root canal or crown work on the remaining teeth.

Diabetes and implants: what HbA1c do we want?

Diabetic patients can get implants, and with good control the results are close to those in non-diabetics. There is no single official cut-off, but research consistently finds HbA1c at or below 7% behaves much like non-diabetic patients, while values above 8% go with more bone loss and gum inflammation around the implant. The trend over months matters more than one report.

The ITI consensus says high implant survival can be achieved in adults with type 2 diabetes. A 2026 systematic review of 54 studies found overall survival often above 90 to 95%, but HbA1c above 8% was consistently linked to more marginal bone loss, deeper pockets and more bleeding around implants. A smaller 2024 review reported survival of 96.1 to 97.3% at one year and 87.3 to 96.1% at five years in well-managed diabetes (HbA1c under 8%). That five-year range is wide, so I treat it as reassuring rather than precise.

What I tell families with a diabetic parent

If the last two HbA1c readings are high, it is usually wiser to spend two or three months with the physician bringing them down than to rush the surgery. The implant is not going anywhere. On the day, your parent should eat a normal breakfast and take their usual diabetes medicines unless the physician says otherwise.

Can someone with high BP or on blood thinners have implant surgery?

Yes, in most cases. Controlled blood pressure does not raise the risk of implant failure, and patients on blood thinners are routinely treated once their physician has been consulted. The rule I repeat to every family is simple: never stop a blood thinner on your own before a dental procedure.

A 2024 meta-analysis of 24 publications compared 4,874 implants in people with hypertension (257 failures) to 16,192 in people without it (809 failures) and found no meaningful difference in failure risk. The ITI consensus similarly reports 98 to 100% survival in patients with cardiovascular disease. So high BP is a planning issue, not a barrier.

Anxiety alone pushes BP up in older patients, and I have seen readings drop by the time the second cup of water is finished. Short sittings and a family member in the room help more than people expect.

Implants vs dentures for a 65-year-old

For a 65-year-old who has lost most teeth, implant-supported options usually chew better and feel steadier than a conventional denture, especially in the lower jaw. A plain denture is cheaper and involves no surgery, so it remains a good answer for parents who are frail or medically unstable.

A 2026 review of 21 randomised trials found that implant-supported overdentures gave older adults better oral-function quality of life than conventional dentures, though the trials varied a lot. The same paper notes that about 30% of adults over 60 worldwide have lost all teeth in at least one jaw. The lower denture is where most complaints come from. It sits on a narrow ridge, the tongue lifts it, and by year ten many parents simply stop wearing it for meals.

When does All-on-4 make sense for a parent?

All-on-4 makes sense when most of the teeth in one jaw are failing or already gone, a removable denture is not being tolerated, and health is controlled enough for one longer surgery. Four implants hold a fixed full-arch bridge that does not come out, which many parents strongly prefer to a denture.

For a full arch, All-on-4 costs ₹3.5–4.5 lakh per jaw here and All-on-6 costs ₹4–8 lakh. All-on-6 is chosen when there is more bone to work with and the bite is heavier. What does a CBCT scan tell us?

A CBCT is a 3D X-ray of the jaw. It shows how tall and wide the bone is where each implant would go, where the nerve runs in the lower jaw and how low the sinus sits in the upper jaw. In older patients who have worn dentures for years, it is often what decides between implants, a graft, or a denture.

If the scan shows thin bone, a bone graft or sinus lift may be needed. That is planned and priced case by case after the scan, never guessed beforehand. Sometimes the scan shows that shorter implants or All-on-4’s angled back implants can avoid a graft altogether, which is a real advantage for a parent who wants fewer procedures.

How do we make the visits easy for elderly patients?

We keep sittings short, book morning slots after breakfast and medicines, and let one family member stay in the room. A single implant usually takes four to five visits over three to four months. Most of those visits are short reviews, and only one of them is the surgery itself.

  1. Assessment: examination, CBCT, medicines and reports reviewed, plan explained to the family.
  2. Surgery: about an hour per implant under local anaesthesia; full-arch work takes longer.
  3. Check-up: about a week later, to see healing and remove stitches if needed.
  4. Healing: three to four months, often with the existing denture adjusted so your parent can eat.
  5. Final teeth: impressions, then the crown or bridge is fitted.
