Aligners for teenagers work well when two things are true: the bite problem is mild to moderate, and your child will actually wear the trays about 22 hours a day. In one study of teens with mild cases, aligners finished faster than braces with fewer visits. But for complex bites, or a teen who will leave trays in a lunch box, fixed braces are the safer call. What Age Are Aligners Suitable From?

For a full aligner course, most teenagers are ready once nearly all their permanent teeth have come through, which in our practice is usually around 12 to 14. The deciding factor is not the birthday. It is whether the last baby teeth are gone and whether your child is mature enough to manage removable trays.

I see two kinds of parents in our Vaishali clinic. One brings a 13-year-old because a cousin got aligners and the child has been asking for months. The other brings a 16-year-old with a crowded lower jaw that nobody noticed until a school photo. Both are reasonable times to come. Our in-house orthodontist will first check which teeth are still erupting, how the jaws are growing and whether the bite is a tooth problem or a jaw problem.

That last distinction matters. If the lower jaw is set far back, a growing teenager may benefit from appliances that guide growth, and that window closes. If it is purely crowding or spacing, there is less of a rush. Crooked teeth are common here: an Indian meta-analysis of 54 studies found malocclusion in 35.4% of children aged 8 to 15.

Aligners vs Braces for Teens: An Honest Comparison

For mild crowding and spacing, aligners can be as effective as braces and a little quicker, with fewer visits and fewer emergencies. Braces win when the bite is complex, teeth need large rotations or pulling down, or the teenager is unlikely to wear trays consistently. Braces work without anyone’s cooperation.

The most useful teen-specific study I know of is Garfinkle and colleagues in the Angle Orthodontist, 2020. They followed 52 teenagers aged 11 to 17 with mild malocclusion. The aligner group finished in 16.9 months against 23.4 for braces, with 13.7 visits against 19.3, and fewer emergency visits. I quote that study often, with a caveat I always add: these were mild cases. Do not read it as aligners being faster for every teenager.   • Better for complex bites, big rotations and impacted teeth
  • More visits, and loose brackets after hard snacks
  • Harder to brush around during exam-season late nights

Will My Teenager Really Wear Aligners 22 Hours a Day?

This is the question that decides everything. Aligner makers recommend at least 22 hours of wear a day, so trays come out only for eating and brushing. A teenager who manages 15 or 16 hours will fall behind the plan, trays stop fitting, and treatment drags on.

The 22-hour figure is the manufacturers’ guideline, as noted in a 2024 survey of 240 aligner patients by Levrini and colleagues. In that same survey, 79.2% said they would even keep aligners in during meals if it shortened treatment. Intentions are rarely the problem. The problem is a Tuesday at 4 pm, a samosa at the coaching centre in Laxmi Nagar, trays wrapped in a tissue, and the tissue in the bin.

What I ask parents to watch for, honestly, before choosing aligners:

  • Does your child already manage a routine without reminders? Homework, a retainer, contact lenses. If yes, aligners usually go well.
  • Do they snack all day? Every snack means trays out, brush, trays back in. Grazers lose hours fast.
  • Are you able to check the trays in the evening? A quick look at dinner is enough. Teens who know someone will ask tend to wear them more.

Trays come out only at the dinner table and go straight into their case, never a napkin. Many families keep a spare case in the school bag and one at home.

What If an Aligner Is Lost or Broken?

Do not panic and do not wait. In most cases your child should go back to the previous tray, or move to the next one if the current set was nearly finished, and you should call the clinic the same week. A replacement tray may need to be made, which takes some days.

Lost trays happen, especially in the first month. The real cost of a lost aligner is not the plastic; it is the days without wear. Before you sign up anywhere, ask how replacement trays are handled and charged, because it varies with the brand and the lab. Get the answer in writing, along with what happens if the plan needs a refinement set at the end.

Should Treatment Avoid Board Exam Months?

Ideally, yes for the start of treatment. The first week of aligners or braces brings soreness and a slight lisp, and the fitting itself is one longer appointment. Starting in the summer holidays or right after a term ends gives your teenager time to settle before exams.

If your child is in Class 9 or 11, that is often the sweet spot: enough time to finish most of the treatment before the board year. Mid-treatment exams are fine. Reviews can be moved a week or two around a Class 10 or 12 board date without harm, and we keep evening and Sunday slots until 8 pm so school is not missed. For sport, aligners can be removed for a match, and teens playing contact sport with braces should use a mouthguard.

When We Would Advise Braces Instead

We recommend braces over aligners for a teenager when the bite needs significant correction, a canine is impacted, teeth are badly rotated or need extractions, or when parent and child both suspect the trays will not be worn. Braces are not a downgrade. For some cases they are simply the better tool.

A few situations where our in-house orthodontist will usually steer a family towards fixed braces:

  • An impacted or stuck canine that needs to be pulled into place. Fixed appliances do this more predictably.
  • Large rotations of round teeth such as premolars, which plastic trays grip poorly.
  • Extraction cases, where gaps have to be closed with roots kept upright.
  • A jaw growth problem that needs growth-guiding appliances first.
  • A child who has already lost a retainer twice. We say this kindly, but we do say it.

What Parents Should Check on the First Visit

On the first visit, look for a full examination with X-rays before any aligner talk, a clear answer on whether the problem is teeth or jaws, and a written plan listing duration, visits, replacement trays and retainers. If someone is selling aligners before looking at an X-ray, that is a warning sign.

  • Were cavities and gums checked first? Teenagers in braces or aligners need healthy teeth to start.
  • Did they explain why aligners, or why braces, for your child’s specific bite?
  • Is the duration given as a range, with what would make it longer?
  • Who handles emergencies, and how quickly can you get a slot?
  • Are retainers part of the plan, and for how long should they be worn?
